Our physical therapists guide you through the following steps with your individualized treatment plan:

We discharge you with a progressive program that will empower you to keep your new, more normal function ongoing.

PREPARING FOR YOUR VISIT

Please arrive 15 minutes early for your initial visit wearing loose, comfortable clothing and plan to bring the following:

Doctor Referral Script

In the state of Texas, our Physical Therapists can evaluate you, but they cannot treat you without a signed referral from your doctor

Insurance Card

Along with Driver's License or another form of photo ID.
